1 Terms and conditions apply for free lifetime take-home whitening after orthodontic treatment: Patients must consent to begin orthodontic treatment, have all their records taken, sign an orthodontic financial agreement and leave a deposit by October 2, 2025 to receive one free whitening gel syringe to whiten their teeth at home at the end of clear aligner therapy. The gel is to be worn inside the orthodontic trays or retainers for 15-30 minutes once a day. To qualify for free lifetime take-home whitening, the patient must be wearing their orthodontic retainers nightly and maintain at least two hygiene visits a year that are at least 4 months apart at Mistry Dental. Patients must also get an annual checkup + x-rays with a dentist at Mistry Dental. All cavities must be treated at Mistry Dental before the patient receives their annual free take-home whitening gel. One free whitening gel syringe will be offered once a year, at least 10 months apart from the last whitening treatment, after all cavities have been treated at Mistry Dental only. The patient must use the gel inside their orthodontic retainers – if the retainers are broken, lost, or no longer fit, the patient is responsible for replacing the retainers at their own cost or purchasing a new set of whitening trays. In order to continue qualifying for free lifetime take-home whitening, patient accounts must be in good standing with no outstanding payments including deductibles, copays, treatment not covered by insurance, or missed appointment fees. If patients receive any dental treatment outside of Mistry Dental (other than treatment that is referred out by Mistry Dental), the free lifetime whitening agreement is null and void and cannot be reinstated, but patients can still purchase whitening gel from our office to use at home with their orthodontic retainers. 2 Terms and conditions apply for free lifetime take-home whitening for new patients: A new patient is defined as someone who has not had any appointment at Mistry Dental in the last 5 years. The new patient must book an intake appointment which consists of a complete oral exam with a dentist, at least one hour of cleaning with a hygienist, and consent to any prescribed x-rays. The new patient must have all active cavities treated before qualifying for free take home whitening. The new patient will consent to impressions of their teeth to fabricate their free whitening trays (valued at $350), and they will receive one free syringe of whitening gel. In order to continue qualifying for free lifetime take-home whitening, the patient must schedule at least two cleanings per year, a minimum of 4 months apart, and get x-rays and a checkup by a dentist. One free whitening gel syringe will be offered once a year, at least 10 months apart from the last whitening treatment, after all cavities have been treated at Mistry Dental only. The patient will receive one set of free whitening trays – these trays must be in good condition and fit well for the patient to continue using the trays with the whitening gel. If the trays are broken, lost, or no longer fit, the patient is responsible for replacing the retainers at their own cost or purchasing a new set of whitening trays.
